Description | Registration card belonging to Janusz Korczak, representing doctors associated with the Warsaw-Bialystok Medical Chamber. This document contains the following data: personal information, date of birth, place of birth, medical specialisation, nationality, marriage status, number of children. Two addresses are given: No 8 Złota St m. 4, where Korczak lived with his sister in the early 1930s and 92 Krochmalna St, where the Orphanage was established and Korczak once again lived at the start of WWII. He gives two birthdates: 22 July 1878 and 1879. This lack of clarity was due to his father – Józef Goldszmit – not completing his birth certificate fully, leading to doubts with this regard for the rest of Korczak's life. This registration card is dated 25 January 1940 – during the occupation, this document had to be filled out by all doctors living in districts belonging to a given Medical Chamber. They also had to complete written declarations relating to their origins, which might explain the absence of this information from the registration card. |
